The Hospice Path of Life Garden is located on the grounds of Humboldt County Memorial Hospital (HCMH). The Path of Life Garden offers a unique opportunity to memorialize a beloved family member or a friend in a permanent way. 

The Path of Life brick pavers are available for purchase. A 4 x 8 single paver brick is available with a $100 donation includes up to 3 lines of engraving. An 8 x 8 double paver brick is $200 with up to 6 lines of engraving.   

Hospice of Humboldt County will be installing new engraved paver bricks in May with viewing after Memorial Day.

Form may also be picked up Humboldt County Public Health, located on the north-side of UnityPoint Clinic - Humboldt or at the front reception desk at HCMH.
